The Case Manager will provide excellent customer service and maintain a caseload of clients to assist them to more effectively function in the community or to prepare them for reintegration into the community.  Duties and additional experience or requirements may vary by position



  1. Maintain caseloads and productivity by ensuring regular contact with clients in community and office.
  2. Monitor and assess progress per agency policy, regulatory requirements and best practices.
  3. Meet with clients individually or in a group to assess needs; collaborate with clients, families and other case managers and clinicians.
  4. May provide but is not limited to: crisis intervention, advocacy, medication monitoring, transportation assistance, financial assistance, ADL assistance, referral and linkage to community resources. May visit, monitor and transport clients.
  5. Prepare clinical reports and individualized service plans, utilizing agency electronic clinical documentation software.
  6. Assure documentation is in compliance with contract, regulatory and agency requirements and best clinical practices.
  7. Efficiently operate personal computer to enter, edit and/or retrieve data and to produce reports and correspondence.
  8. Effectively work as a team member including participating in all required trainings, ensure the safety of others and participate in Peer Review/CQI process.

  1.  Bachelor’s degree in Behavioral Science or related area or
  2.  Equivalent combination of experience and training
  3.  A valid driver’s license and current auto insurance may be required.
  4.  Energetic support of the overall vision and mission of the Greater Cincinnati Urban League.
  5.  Good oral and written communication skills.
  6.  Must have the ability to perform accurate and timely data entry while demonstrating data       integrity.
  7.  Knowledge of Business English including grammar, punctuation, and spelling.
  8.  Must be proficient in Word, Excel, and PowerPoint software applications.
  9.  Skilled in the maintenance of database systems.
  10.  Ability to work flexible schedule, including evenings and weekends.
  11.  Ability to interact with internal staff and external customers.
  12.  Dynamic interpersonal skills.
  13.  Display high level of professionalism and impeccable integrity and confidentiality.
