The Greater Cincinnati Urban League has been providing case management services through our Community Access Coaches to Avondale Choice Neighborhood residents since June, 2013. The Urban League describes case management services as an individual or family-centered approach to assisting people with accessing the services they want and need. These services include but are not limited to; screening, risk management assessments, creating individualized services plans, linkage/referrals to services and supports, monitoring resident progress, advocacy, follow-up, and maintenance of records. The A.C.N. Urban League Staff currently consists of a Program Manager, two professional Community Access Coaches, and a Data Manager.
